In a case reported by Larkin et al. (2025), a patient exhibited symptoms of both myasthenia gravis and myositis, which is muscle inflammation, without prior exposure to immune checkpoint inhibitors, drugs known to cause such overlap syndromes. The patient's symptoms included muscle weakness, eye movement issues, difficulty swallowing, and respiratory failure due to neuromuscular issues. Initial treatments with steroids and intravenous immunoglobulin provided partial relief. A significant improvement was seen with the use of rituximab, a medication that targets immune cells, and an acetylcholinesterase inhibitor, which helps improve muscle strength.

Despite no initial evidence of a thymic mass, which can be associated with myasthenia gravis, the patient underwent a thymectomy, the surgical removal of the thymus gland. The procedure confirmed the presence of a thymoma, a tumor originating from the thymus. This case underscores the importance of considering that unusual disease presentations might be due to the simultaneous occurrence of two conditions. It also highlights the need to consider paraneoplastic causes, where a tumor causes symptoms not directly related to the tumor's presence or spread.
